New Patient Registration
We can only accept new patients who live in our practice area. You can register by completing a registration form which can be obtained from the practice or downloaded from the above “Registering Online” tab.
As a patient you have the right to express which particular practitioner you may wish to consult. The practice will also require some form of identity, e.g. marriage or birth certificate, driving licence, paid utility bills or evidence of benefit entitlement.
When you register, we will ask you to make an appointment to see the doctor followed by a new patient check with our practice nurse.
Please note that patient registration takes up to 48 hours to complete and therefore in an emergency please contact your previous surgery or the nearest walk-in Centre.
